Most likely is an anal fissure. This is a small, linear wound in the anus produced usually due to straining or constipation. It usually hurts a lot during defecation and may leave blood streaks in the stools. You may use a hemorrhoidal cream for some days (like preparation H), but the most important thing is not to be constipated, because it won't heal. You can use Colace 1 tablet 2 times daily for some days and eat plenty of fiber (raisins, granola) to have bowel movements. Fiber may take however 1-2 days for effect, so may use Milk of Magnesia to have 1-2 soft bowel movements per day. Sitz baths also help a lot and keep the anal area clean. It'll go on its own after some days.
